who, because of mental or physical impairment, cannot remain alone during
the day. A stimulating atmosphere aimed at encouraging the individual to
utilize his or her capabilities to the maximum.
A place of socialization, where the effects of isolation can be prevented. |

Leewood Adult Daycare Center
operates Monday through Friday from 7:00 a.m. to 6:00 p.m.
The daily rate is $50.00.
Participants may attend for two to five days a week. |

| Requirements
for Admission |
|
Leewood Adult Daycare Center is licensed by the
Virginia Department of Social Services.
Our admissions process, therefore,
must conform to state regulations. |
|
Prior to Admission
 | A personal interview is held in the
center, with the applicant and appropriate members of our staff. The
interview is an informal conversation during which all parties may observe
the applicant’s appropriateness for adult daycare. It also enables the
applicant to see for himself/herself what adult daycare offers. |
 | A physical examination form must be
completed by a physician approved by Leewood, within 30 days prior to
admission or acceptance for admission. |
 | A tuberculosis screening, according to
the accepted methods of the Virginia Department of Health, must be done
within 30 days prior to admission or acceptance for admission. |
 | Medical forms must be approved by the
Center’s registered nurse. |
 | A local personal physician, licensed to
practice in the Commonwealth of Virginia, shall be named by the applicant.
This physician shall agree to be available for consultation with the
Leewood staff as necessary, and must authorize all medications. |
 | Completed information forms must be on
file prior to admission. Much of this information is required by state
regulation. |
At the time of Admission
 | Completion of the admission contract
and other forms, such as field trip permission, which takes an average of
30 minutes. |
 | Completion of agreement forms to be
signed by the participant when necessary. |
 | All participants are admitted on a trial basis
for a period of one month. |
 | Agreement that the participant shall
attend at least two days weekly. |
