programs.

| Requirements
for Admission |
|
Leewood Healthcare Center is licensed by the Virginia
Department of Social Services.
Our admissions process, therefore, must conform to state regulations.
|
|
Prior to Admission |
 | A physical examination form must be completed by a
physician approved by Leewood, within 30 days prior to admission or
acceptance of admission. |
|
 | A tuberculosis screening, according to the accepted
methods of the Virginia Department of Health, must be done within 30 days
prior to admission or acceptance of admission. |
|
 | Medical forms must be approved by the Center's
registered nurse. |
|
 | A local personal physician, licensed to practice in the
Commonwealth of Virginia, shall be named by the patient. This physician
shall agree to be available for consultation with the Leewood staff as
necessary, and must authorize all medications. |
|
 | Completed information forms must be on file prior to
admission. Much of this information is required by state regulation. |
|
|
At the time of Admission |
 | Completion of the admission contract and other forms,
which takes on average of 30 minutes. |

| New Family Orientation
Sessions |
|
We encourage
you to join us for a “New Family Orientation” session.
These meetings are held the first Tuesday of the month at 9:00a.m. &
the third Tuesday of the month at 6:30p.m. They are an excellent opportunity for you to get answers to your questions
about life at Leewood.
The sessions are informal, but we do ask that you RSVP so that we can plan
for your attendance.

| Resident's Council |
|
The purpose of Leewood’s Resident’s
Council is to empower our residents to be involved in the day-to-day issues
of the facility. It is an excellent forum for residents to make suggestions
for improvements and to receive regular updates on what is happening at
Leewood.

Meetings are held the second Tuesday of each month
at 10:00 a.m. All residents are invited to attend.

| Leewood Family
Council |
|
The Leewood
Family Council is a group of family members and friends of loved ones who
reside at Leewood.
The council was established in 1987 to actively promote a
quality lifestyle for each and every resident and maintain a cooperative
relationship with administration, staff, and all others who are in pursuit
of this mission. The goal of the council is to assist and insure that
ongoing quality care is provided to all residents. Family Council has made a
positive difference in the lives of residents at Leewood, and has been
instrumental in encouraging improvements in the past five years. The council
can help you to help your loved ones by suggestion how to resolve problems
and by providing a strong, united voice of support.
The council meets on the first Wednesday of each month in the
Virginia Room at Leewood from 7:00 to 9:00 p.m.
We invite speakers,
discuss issues, and provide support for families.
Your participation is
essential, and we
encourage you to join us and help the council
continue to
make a difference in the lives of Leewood residents.
